will be sent from the Registrar's Office
mid-semester of your graduation term.

To apply for graduation follow these steps:
1. Print and complete the CBT
Graduation Application.*
- Be sure to print and complete both pages of the application.
- All students applying for graduation will have a $20 fee
added to their EKU account for each diploma.
- Note that it is your responsibility to notify the CB&T Advising Office of any name, address, phone, and e-mail changes or if you plan to change your date of graduation in order to ensure that you receive your final transcript and diploma.
2. Complete the online survey(s). Click the hyperlink below, read the online instructions and complete the survey(s) as directed. Submit your completion verification form(s) with your graduation application materials.
The following two surveys must be completed ONLY by all teaching majors that require admission to the Teacher Education program:
3. Print and complete the EKU Alumni Association Information
Sheet.
4. Select and proceed with one of the following options:
Option 1: Submit graduation documents via fax (859-622-1382), or in-person delivery to BTC 257.
- Confirmation of receipt for your graduation application will be provided via the email address indicated on your graduation application. If you do not receive confirmation within one – two weeks, you must contact our office.
- Advisors will review your application documents and contact you as needed via the email address or phone number indicated on your graduation application.
Option 2: Call 859-622-1086 to schedule an exit interview if an in-person appointment is preferred.
5. Remit the documents completed in steps 1-3 using either Option 1 or Option 2 above:
- Graduation Application
- Online Survey Verification Form(s)
- Alumni Association Information Sheet
*Attention CTE Majors: Students receiving a B.S. degree in Career
& Technical Education must send their Graduation Application documents
to Dr.
Stephen Fardo. |
